§ 54205. Physician Request.
(a)
Adult day health care services shall be requested in writing by a physician. The request shall include:
(1)
Principal and significant diagnoses.
(2)
Prognosis.
(3)
Specific type of treatment and anticipated duration.
(4)
Overall therapeutic goals for the participant.
(5)
Medications and special diets.
