§ 100147.02. STEMI Receiving Centers.
(a)
Eligibility. A licensed general acute care hospital (GACH) meeting the requirements of this section may be designated by the local EMS agency (LEMSA) as a STEMI receiving center.
(b)
Requirements.
(1)
Capabilities.
(A)
Twenty-four (24) hours per day, seven (7) days per week, 365 days a year availability for the treatment of STEMI patients;
(B)
A call activation system to activate the cardiac catheterization team directly;
(C)
An EMS system notification process developed with the LEMSA to be utilized when PCI services are unavailable;
(D)
A process in place for the triage and treatment of STEMI patients arriving simultaneously;
(E)
Compliance with the required minimum volume of PCI procedures performed for designation by the LEMSA;
(F)
A STEMI receiving center without on-site cardiovascular surgery capability shall have a written transfer plan and agreements for transfer to a facility with cardiovascular surgery capability.
(2)
Program Staffing.
(A)
A STEMI program manager and a STEMI medical director;
(B)
A cardiac catheterization team, including appropriate staff as determined by the GACH and LEMSA, that is immediately available at all times;
(C)
Up-to-date STEMI team and cardiac catheterization team call roster(s);
(D)
Documentation containing job descriptions and organizational structure that clarifies the relationship between the STEMI Medical Director, STEMI Program Manager, and the STEMI team, to be provided to the LEMSA upon request.
(3)
STEMI Treatment and Operations. Agreement to:
(A)
Accept all STEMI patients according to the LEMSA's STEMI destination policy;
(B)
Adhere to the LEMSA's diversion policy or protocol.
(4)
Policies. Written policies and protocols for triage, diagnosis, and cath lab activation following field notification.
(5)
Continuing Education. Continuing education in STEMI critical care shall be provided for:
(A)
Organized medical staff;
(B)
Staff nurses;
(C)
Staff allied health personnel;
(D)
EMS personnel; and
(E)
Other community physicians and health care personnel.
(6)
Data Collection, QI, and Evaluation. STEMI receiving centers designated under this section are subject to the requirements of Article 5 of this subchapter. In addition, STEMI receiving centers shall have a review of its STEMI program conducted by the LEMSA, in accordance with LEMSA policies and procedures, or through a nationally recognized STEMI program, depending on the certification level, at least every three (3) years to ensure the program continues to meet state minimum standards as well as local and certification requirements. The review shall include, at a minimum, the evaluation of documentation, credentialing records, and compliance with applicable standards. Documentation of the review and determination shall be included as attachments with the Initial STEMI Critical Care System Plan Template (Rev. 11/2025), incorporated by reference, or the Annual STEMI Critical Care System Plan Update Template (Rev. 11/2025), incorporated by reference, as applicable.

Additional Requirements. A LEMSA medical director may establish requirements for designation as a STEMI receiving center in addition to those required under this section.
(c)
STEMI Receiving Centers Designated Prior to Effective Date of Regulations. A STEMI receiving center designated by the LEMSA pursuant to regulations in effect prior to the effective date of these regulations may continue to operate as a STEMI receiving center; however, any such STEMI receiving center shall meet the requirements of these regulations within twelve (12) months following a STEMI program review described in subdivision (b)(6) of this section.
